Office Policies

Appointments

You may schedule an appointment by calling our office. If you need to reschedule an appointment or if you will be late for your appointment, we ask that you call as soon as possible so we can call our patients on the waiting list. Please bring your insurance card with you to the office during the time of your appointment if you are a new patient or have just received new insurance. If you are a new patient, you may print the New Patient Information Form that is located under the New Patients tab and fill it our prior to your visit to expedite the waiting time in the office.

After Hours

Please call the office and follow the directions on the voice mail.

Billing

If we are contracted with your insurance carrier, our office will file your insurance claims directly to your health insurance carrier. If you should happen to change your insurance during the course of treatment, please communicate this promptly to our offices to avoid any unnecessary bills to you. All copays are due at the time of service. Our office accepts cash, Visa, MasterCard, personal check, money order.

If you have a question regarding your account, please contact the office where you were seen.

Disability Forms

If you have a disability form that needs to completed, please remember that it can take up to fifteen (15) business days for processing. There is a $25.00 paperwork fee for completing the forms.

X-Ray Requests

Physicians are required to maintain medical records including X-Rays for 7 years. Therefore, X-Rays are a part of your permanent medical record. If you would like to receive a copy of X-Rays, you can purchase them for $5.00 per sheet/CD. Please allow a minimum of 5 business days for processing your request.

Medical Record Requests

All medical records requested must be authorized by our patient and made in writing by a formal written release. A copy of the Authorization to Release Records is available to you in each office. All record requests will be processed within 30 days of the written request. A processing fee of $1.00 for the first page and $0.50 per page thereafter plus postage will apply unless it is requested by a licensed physician by any state for the purpose of emergency or acute care.

Test Scheduling

If you were indicated for a test (MRI, CT, Nerve Conduction Study, etc.), our office staff will make the appointment for you and contact you immediately. However, on many occasions, test scheduling cannot be completed until the office notes have been transcribed and sent to your insurance company. As a result, this may delay your test scheduling.

Test Results

Upon scheduling your ordered test, you will be scheduled for a return appointment. Our physicians will discuss test results with you during your return appointment.

Prescription Refills

To facilitate the processing of your prescription refills, please call your pharmacy for any refill requests. The pharmacy will call our offices with any questions regarding your refill. If you are requesting a new medication or a different medication from what you are taking, please call our offices directly and speak with our staff who will ask the physician. To process any new prescriptions efficiently, please have your pharmacy number available.

If you are taking a pain medication, you must be seen by our physicians at least every 3 months in order to obtain a refill.

If you are taking an anti-inflammatory medication, you must be seen by our physicians at least every 6 months in order to obtain a refill.

Our office will attempt to process all prescription requests within 24 hours unless Prior Authorization from your insurance company is needed. Refill requests will not be processed after office hours or over the weekend.
